TERMS TO KNOW COPAYPREMIUMDEDUCTIBLEIN-NETWORKCOINSURANCEOUT-OF-NETWORKPOINT OF SERVICE (POS) PLANPRIMARY CARE PROVIDER (PCP)HIGH DEDUCTIBLE HEALTH PLAN The amount you pay for a health plan in exchange forcoverage. Health plans with higher deductiblestypically have lower premiums. The amount or percentage that you pay for certaincovered health care services under your health plan. Thisis typically the amount paid after a deductible is met andcan vary based on the plan design.A flat fee that you pay toward the cost of coveredmedical services. The amount can vary by the type ofservice.A specific dollar amount you pay out-of-pocket beforeyour benefits covered by your insurance plan. Undersome plans, the deductible is waived for certainservices. Healthcare physicians or facilities that are in yourinsurance plan’s network. When you receive in-networkcare, your savings and coverage will be greater thanusing out-of- network services!When selecting care, members should try to avoid out-of- network providers or facilities since the costs of carecan be high. This is because out-of-network physiciansand facilities do not have a contract with your healthinsurance plan provider. A Point of Service (POS) Medical plan gives membersaccess to a vast network of doctors and hospitals andprovides coverage for preventive care, doctor visits,hospital stays, and other services you may needthroughout the year. Members can take advantage ofcost savings by seeking in-network care.A health plan whose deductibles cannot be below theIRS minimum limits to allow participants to contributeto a Health Savings Account. The monthly premium isusually lower than traditional PPO plans, but you donot have the convenience of copays and may paymore out of pocket for health care services before theinsurance company starts to pay its share (yourdeductible). A high deductible plan (HDHP) can becombined with a health savings account (HSA),allowing you to pay for certain medical expenses withmoney free from federal taxes.A primary care provider, also known as a primary doctor,is your main health care provider in non-emergencysituations since appointments are required. All participating employees must contribute 5% of their regular earnings to the plan, and the institution contributes 10% ofeach participant’s regular earnings. Employees may elect to contribute extra amounts (beyond 5% of their earnings) towardthe purchase of larger TIAA annuities, but Berry does not match additional payments.Your employer offers you three voluntary medical supplemental plans!
