This will be the number printed on your ID card.Quantum Health will begin operations December 1, 2024 to addressmember questions. WellDyne: New RX provider and new formulary (drug list). The newformulary will only apply January 1 to brand new prescriptions. Existingprescriptions will be honored for 90 days to allow you time to consult withyour physicians about changes in medications. Aetna (Meritain): New claims payor and provider network (Aetna ChoicePOS II Network). Aetna providers currently service Polk County Government.Mail-order Pharmacy: Changed to mirror 90-day retail supply copay tocomply with new Florida law.Lantern Surgery Care: New elective surgery benefit provided at no cost tohealth plan members. More details will be shared in the coming months. ID Cards: Everyone will receive new ID Cards in the mail. Please make sureyour address is up to date in the Staff Portal. Board Spouse: If you are a Board Spouse, please contact Risk Managementto change your election as this benefit can only be changed by RiskManagement staff. Not making changes? If you are not making changes to your coverage, yourcurrent election will automatically carry over to next year. Transition of CareMedical: Are you in the middle of a treatment plan? Quantum Health willbe receiving information from BCBS and assisting with transition of care toensure continuous care. Pharmacy: A file containing information on current refills remaining at theend of December from Prime Therapeutics will be provided to our newpharmacy provider, WellDyne, to allow you to refill medications.Questions?
