Enroll Now This year’s enrollment will be passive with the exception of FSA and HSA elections.Teammates MUST elect these benefits during Annual Enrollment if you would like coverage in 2025.*You must request a change to your benefits within 30 days ofyour life event (60 days for changes involving Medicaid eligibility). Documentation will be required.At Annual Benefits EnrollmentIf you have a Qualifying Life EventEligibilityMaking Benefit SelectionsFor youIf you are an active regular teammate, working an average of 30 or more hoursper week, you are eligible to enroll in benefits.Enrolling in CoverageYour benefit plans are in effect January 1 – December 31 each year. In general,there are two times you can make benefit selections:You may also cover your eligible dependents when you elect coverage foryourself.Your SpouseAnnual Benefits Enrollment is your annual chance each year to reviewyour coverage options and make changes to your benefits.Your choices are in effect from January – December of the followingyear unless you have a Qualifying Life Event.Qualifying Life Events allow you to change your coverage during theyear outside of Annual Enrollment.* These include:marriage or divorce, birth or adoption, death of a covered dependent, and a change in eligibility through Medicare, Medicaid, or a spouse orparent's coverage.Your ChildrenYou may cover your legally married spouse. Important Note: If you currently cover your spouse on an AIT medical plan andthey have medical coverage available elsewhere, you can be eligible for up to$1,200 annually deposited into a Lifestyle Spending Account (LSA) if you removethem from the AIT plan during Annual Benefits Enrollment and enroll them inother coverage. See page 6 for more information. Dependent children are eligible: Your (and your spouse’s) dependent children up to the age of 26 (formedical, dental and vision plans). Disabled children over the age of 26 ifunmarried, incapable of self-support, dependent on you for primary supportand the disability occurred before the age of 26.Children include your: natural and legally adopted children, children whohave been placed with you for adoption, stepchildren, foster children andchildren for whom you are the legal guardian.Covering your family4Teammates must enroll in benefits via Paylocity: https://access.paylocity.com.

Balance billingWhen you use an out-of-network medical or dental provider, they may bill you thedifference between what they charge and the amount your insurance pays.Medical: balance billing is in addition to – and does not count towards – your out-of-pocket maximum.DeductibleThe amount you’re responsible for paying in care expenses before the plan startspaying deductible-eligible expenses.CoinsuranceAfter you’ve met your deductible, you’re sometimes responsible for a percentage ofthe cost of the medical care, dental care, or prescription medication you received.This percentage is coinsurance.CopayA flat fee you pay each time you receive a copay-eligible medical, dental, or visionservice or prescription medication. Embedded deductible vs. Non-embedded deductibleEmbedded: Each family member has an individual deductible in addition to theoverall family deductible. If an individual in the family reaches his or herdeductible before the family deductible is reached, his or her services will be paidby the insurance company.Non-embedded: There is no individual deductible. The overall family deductiblemust be reached, either by an individual or by the family, in order for theinsurance company to pay for services.In-networkIn-network care is always your lowest-cost option. Networks are groups of medical,dental, and vision providers, pharmacies, and facilities that agree to discount the costof their care or service.Out-of-NetworkOut-of-Network care is your highest-cost option. Health care providers or servicesare not contracted with your insurance company and do not offer discounted pricing.Out-of-pocket maximumThe most you’ll pay for covered in-network medical care in a year. This includes yourdeductible, any coinsurance or copays, and prescription drugs. The out-of-pocketmaximum does not include your premium (the amount you pay for coverage), non-covered expenses, or out-of-network care that’s been balance billed.Pre-authorizationA decision by your health insurer or plan that a health care service, treatment plan,prescription drug or durable medical equipment is medically necessary. Your healthinsurance or plan may require preauthorization for certain services before you receivethem, except in an emergency.Primary care physicianA primary care physician (PCP) is your main medical doctor – usually a generalpractitioner (GP), family doctor, internist, OB/GYN, or pediatrician (for children).ReferralSome specialty medical providers and services require a referral from a primarydoctor. Take advantage of the opportunity to optimize your wallet!6We understand that having your spouse enrolled in medical coverage can be costly, regardless of whether theyutilize the benefits. There may be more affordable options available through another employer or other sourcessuch as Medicare and Healthcare.gov, etc. that could better meet your needs.We encourage you and your spouse to thoroughly evaluate these alternatives to ensure you are making the mostinformed decision. You could be missing out on an opportunity to save in reduced premiums and as an additionalincentive, we will provide up to $1,200* in a LSA (Lifestyle Spending Account), to pay for wellbeing related itemsand activities.To be eligible, you must drop your spouse from coverage during 2025 Annual Enrollment and submit proof ofenrollment in other coverage through AITConnect.*Deposited in $100 increments on a monthly basis. To receive the full $1,200, spouse must be enrolled in AIT Medical in 2024, spouse must be dropped from AIT Medical during 2025Annual Enrollment, and proof of other Medical coverage must be provided in January 2025. If your spouse is added to AIT Medical coverage mid-year due to a Qualified Life Event,you will no longer be eligible and no future contributions will be made.Example: Teammate + Spouse to Teammate Only CoverageIn 2024 Steve enrolls himself and his spouse into the AIT Health Prime PPO Plan. For 2025 Steve elects toremove his spouse from AIT's Medical Plan because his spouse has coverage elsewhere. Medical OverviewFind Health Prime and HealthSavings Plan ProvidersFind Health Advantage PlanProvidersMedical coverage offers health care protection for you and your family.AIT realizes that each teammate’s medical needs vary. Therefore, AIT offers a choice of three medical plans, togive you the flexibility to choose a plan that is right for you and your family. While the plans offer different levelsof coverage and cost, they have an important similarity — a focus on prevention. All three plans areadministered by Aetna and offer preventive care at no cost when using in-network providers. All three plans alsooffer similar covered services and a national provider network. The main difference is what you pay for thatcoverage.Health Advantage Plan with HRA & Supplemental HealthThe Health Advantage Plan is a PPO that leverages copays for routine services. To help offset out-of-pocketexpenses, a company-funded Health Reimbursement Account (HRA), as well as supplemental coverage(accident, critical illness, and hospitalization insurance) is included. The Health Advantage Plan offers lowerpremiums than the other two options. For more information on the HRA, go to page 9.Health Savings Plan with HSA 7Health Prime PlanThe Health Savings Plan is a PPO that comes with a Health Savings Account (HSA) to help pay for current orfuture out-of-pocket expenses. AIT helps fund the account, and you can also contribute pre-tax to your HSA totake advantage of payroll tax savings. The Health Savings Plan features lower premiums than the Health PrimePlan. Out-of-Network services will not be covered starting in 2025, except in certain emergency care situations.For more information on how the HSA works, go to page 11.The Health Prime Plan is a more traditional PPO plan offering the predictability of copays for most services andprescriptions. It offers a lower out of pocket maximum and lower individual and family deductibles. Thepremiums for this plan are higher than the other two medical plans. Out-of-Network services will not be coveredexcept in certain emergency care situations. See page 8 for more details on this plan’s coverage.

Find RxAetna uses CVS Specialty to manage specialty medications and connect you with a care team that hasexpertise in your condition. Your team — nurses and pharmacists who are specially trained in yourcondition — helps you understand how to use your medicine. They’ll also:Remind you when it’s time to refillHelp you stay on track with your treatmentHelp you manage symptoms and side effectsAlong with the Specialty program, CVS Specialty provides a co-pay program to members to secureavailable copay assistance for specialty drugs through the various programs funded by pharmaceuticalcompanies. This program is called PrudentRx Solution.Pharmacy Overview8PharmacyAll three medical plans include prescription coverage. You can fill your prescription drugs through any retailpharmacy. For maintenance medications (ones you take often), you may pay more at retail! For a lower costalternative, Aetna uses CVS Caremark Mail Service Pharmacy for mail order prescriptions. You can get yourmedicine sent conveniently to your home — or anywhere you choose – with a three months supply at the priceof two. Also available at CVS pharmacies is a 90 day supply of maintence medication for the same price asmail order.
