OPEN ENROLLMENT Open enrollment runs each spring. All employees mustre-elect benefits for the upcoming year in EmployeeNavigator during this time. Click here to enroll in yourbenefits. Company ID: CCWA2022New benefits will be effective every May 1.NEW HIRE ENROLLMENTNewly eligible full-time employees are eligible as of thefirst of the month following their first day of full-timeemployment. Variable-hour employees are eligible within30 days after the applicable 12-month testing period(plus any administrative period) if the employee hasworked 1,560 hours during the testing period.MID-YEAR CHANGESAfter your benefits start, you can only change yourbenefits elections during the plan year if you or youreligible dependents experience a qualified mid-yearchange. Examples may include getting married ordivorced, having a baby or adopting, and gaining orlosing coverage. You must notify your OneDigital ClientAdvocates within 30 days of the mid-year event to beeligible to change your elections. ELIGIBILITYAll full-time employees are eligible to elect benefitsthrough CCWA’s plan. Spouses and dependent childrenof the employee are also eligible to participate in thebenefits plan. 4 I Clayton County Water Authority Benefits GuideTERMS TO KNOWCOINSURANCEHow the cost of a health ordental expense is sharedbetween you and the plan afteryou pay your deductible.SUMMARY PLAN DESCRIPTION (SPD)A document that contains a description of the company benefit plans and includes such information as eligibility forparticipation, schedule of benefits, benefit descriptions, plan exclusions, and claim provisions. DEDUCTIBLEThe amount of money you must pay toward health or dental expenses for eachfamily member every plan year before health or dental benefits are payable.Deductible amounts vary according to the benefit plan. After you have paid yourdeductible, future expenses are covered at the coinsurance amount.Copayments do not count toward the deductible. OUT-OF-POCKET MAXIMUMThe most you will have to spend every plan year for each covered familymember for the annual deductible and your coinsurance. Once you’ve met theout-of-pocket maximum on yourself or a covered dependent, the plan pays100% of most remaining expenses for you or your dependent for the rest of thatplan year up to the lifetime maximum.COPAYA set dollar amount you pay foran office visit or prescriptiondrug. Your insurance carrier will help you pay for preventive care, routine health needs,prescriptions, and advanced procedures. For the benefit plan year, Clayton County Water Authority is giving you theoption to choose between one of four different medical plans. Clayton County WaterAuthority provides eligible employees and their dependents with Basic Life and Accidental Death andDismemberment insurance. This benefit will provide financial support in the untimely passing of a coveredparticipant or if a participant is affected by a debilitating injury. Please be advised that at age 70, your coverage willbe reduced by 35%. At age 75, your coverage will be reduced by 50%.Purchasing Voluntary Life Insurance for yourself and your eligible dependents can provide additional financialsupport in the untimely passing of a covered member. Employees can purchase this benefit for themselves, theirspouse, and their eligible children. The cost is based on age banded rates and will change as you age into the nextage band. Please be advised that at age 70, your coverage will be reduced by 35%.
