2023-2024 Employee Benefits Guide 4When Coverage Begins New hires become eligible for coverage on the first day of the month following their date of hire formedical, dental, vision, FSAs and all voluntary benefits are available during open enrollment. If a new hirestarts on the first of the month, coverage will be effective that day.When you enroll during Open Enrollment, coverage begins the first day of the plan year, October 1.Once you make an election, it is in effect for the entire plan year and can only be changed at the next OpenEnrollment period or during the plan year if you or your eligible family member experiences a life event thataffects your eligibility for coverage.Making Changes to CoverageOnce you make your benefit elections, these choices remain in effect until the next annual Open Enrollmentunless you have a qualified family status change, or you or your eligible dependents become eligible forcoverage through special enrollment rules. If you experience a life event that affects your eligibility forcoverage, you may be able to change your benefit election during the plan year.Qualified Family Status Changes Include, But Are Not Limited To:Life events include:▪ Change in marital status▪ Change in number of dependents due to birth, adoption, death, or divorce▪ Change in employment status for yourself or your spouse▪ Special enrollment rights, such as marriage, birth, adoption or loss of other coverageChanges to your benefit elections must be consistent with the related status change and documentation maybe required. For example, when your head cold turns out to be the flu, or when your son’s football game endswith a broken leg, that is when Colonial Beach Public School's benefits are there to help you.This guide provides an overview of our benefits program, which gives you the coverage you need for all typesof things life brings your way. Colonial Beach Public School's benefits allow you to choose the plans that workbest for your own needs – and your pocketbook. The key to getting the most from our benefits program is totake an active role in understanding and using the plans so that you are getting the best value for the moneyyou spend.Who is Eligible?You are eligible for the benefit plans if you are regularly scheduled to work at least 30 hours per weekDependent EligibilitySome benefit plans require you to make an election to enroll. If you make an election to enroll, you may alsocover your eligible family members in the health plan and other plans with dependent coverage. Your eligibledependents are defined in each benefit policy and include:▪ Your legal spouse (from whom you are not legally separated or divorced)▪ Unmarried/married dependent children (including stepchildren, legally adopted children andchildren placed with you for adoption and guardianship) to their 26th birthday, regardless ofstudent or residence status.▪ Unmarried/married dependent children (not their spouse or dependents) who are physically ormentally disabled. Proof of disability is required.Benefits TerminationIf your employment with Colonial Beach Public Schools ends, medical, dental and vision benefits end on thelast day of the month. 2023-2024 Employee Benefits Guide 6Key Health Insurance Terms▪ Copayments – A flat fee you pay for certain covered services, such as, doctor visits, urgent care visits,emergency room visits or prescriptions.▪ Deductibles – A flat dollar amount you must pay out of your own pocket before your plan begins to pay forcertain covered services.▪ Coinsurance – After you’ve reached your deductible, you and your plan may share some of your medicalcosts; the portion of covered expenses you are responsible for is called coinsurance.▪ Out-Of-Pocket Maximum – Specific limits for the total amount you will pay out of your own pocket; onceyou meet this maximum your plan then pays 100% of medical and prescription charges for the remainder ofthe plan year. Please be aware that this does not apply to out-of-network balance billing.▪ Health Savings Account – A Health Savings Account (HSA) is an account created for individuals who arecovered under High Deductible Health Plans (HDHPs) to save for medical expenses that HDHPs do notcover. Contributions are made into the account by the individual or the individual's employer and are limitedto a maximum amount each year. The contributions are invested over time and can be used to pay forqualified medical expenses, which include most medical care, such as, dental, vision and certain over the-counter drugs.▪ In-Network – Accessing care from a provider contracted with the insurance plan you have selected offersricher benefits and lower out-of-pocket costs.▪ Out-Of-Network – Accessing care from a provider that is not contracted with the insurance plan you haveselected may result in higher out-of-pocket costs.▪ Emergency Room – Meant for true emergencies to provide fast, life-or-limb saving care for severe and life-threatening conditions. Emergency Rooms are equipped for nearly any type of medical emergency. Theyhave the widest range of services for emergency after-hours care, including diagnostics tests and access tospecialists. Emergency Rooms are found at most hospitals, and they are typically open 24 hours a day, 7days a week.▪ Urgent Care Center – Set up to assist patients with an illness or injury that does not appear to be life-threatening, but also can’t wait until the next day or for their primary care physician to see them. A trueurgent care center will be staffed with doctors and nurses who have access to x-ray and a lab onsite. Mosturgent care centers are open after normal business hours, including evenings and weekends.Enrollment is due within 31 days of your qualifying event date. You choose the option thatmakes the most sense for you and your family basedon your needs and what you want to pay for.Choosing a Medical OptionWhen it comes to medical coverage, Colonial BeachPublic Schools provides you with the choice of threeAnthem BCBS medical plans. Plan options includethe HealthKeepers HSA 2000, HealthKeepersPOS 1000, and KeyCare 30 2000.Plan OverviewWith these plans, you can see any In-Networkdoctor, including specialists, without referrals. Youdo not need to choose a PCP; however, we dosuggest you have a PCP to help manage your care.If you choose to receive care outside the network,you will have a significantly lower level of coveragewhich means you might have to pay more for theservice.In an emergency, your care is covered. The FSAs are administered by Aflac/HealthEquity. Click here to learn more about Flexible Spending Accounts.How the FSAs work You are offered two types of FSAs:▪ Healthcare FSA can be used to pay for most out-of-pocket medical, vision, and dental care expenses for yourself and your eligible dependents. ▪ Dependent Care FSA can be used to pay for eligible expenses for the care of a dependent child up to age 13 or a dependent adult. If you elect to contribute to the Healthcare FSA and/or the Dependent Care FSA, you choose an annual amount to be taken from each of your paychecks and deposited into your account throughout the year. Your contributions are taken out of your paycheck before you pay taxes. Then, when you have eligible health care or dependent care expense, you can use the account to reimburse yourself, up to the amount you have elected to contribute to your account for the year. Your contributions are subject to annual IRS contribution election to ensure that your total contributions to the FSA do not exceed the calendar year annual limit imposed by the IRS. Annual Contribution Amount For 2023, you can contribute up to $3,050 to the Health Care FSA. You can contribute up to $5,000 to the Dependent Care FSA. As a reminder, your benefit elections are made on a plan year basis and cannot be changed mid-year without a qualifying life event. Health Care FSAYou can use the Health Care FSA to pay for eligible out-of-pocket expenses that are not covered by another health plan.
