Frequently asked employee questions related to Voya Flexible Spending Accounts General Who is Voya? Voya Financial, Inc. (NYSE: VOYA), helps Americans plan, invest and protect their savings — to get ready to retire better. What do I do if my debit card declines or doesn’t work? Ensure you have activated the card and have logged into the online portal to accept your terms and conditions. Within your profile, confirm you have the correct and most recent card by verifying the last four digits. Check your balance and verify via your activity that there are no pending transactions you have not accounted for. If you are using the card at a retail pharmacy, ensure the item is coded as FSA eligible. Still unable to determine the cause? Contact the Voya Health Account Services team at (833) 232-4673 or HASinfo@voya.com to verify the cause and best solution. Once I request reimbursement, how long does it take for my funds to be available? In general, if you have a bank account linked to your account this can take as little as 3 business days for the reimbursement to be posted. However, a check can take up to 7 business days to arrive. Reimbursement is dependent on if the claim documentation is approved for FSA and Commuter accounts. How can I grant my spouse access to my account? You may submit a Power of Attorney Form or an Authorized Representative Form. These forms can be found via the consumer portal or through your employer. Can I use my FSA to pay for medical expenses for a spouse or dependent? Yes, you may use your FSA funds without penalty to pay for qualified medical expenses for yourself, spouse, or dependent even if they are covered under another health plan. Flexible Spending Accounts (FSA) What are the current IRS contribution limits for FSAs? The IRS sets contribution limits annually. The 2023 maximum annual FSA contribution limits are as follows: Health FSA - $3,050; Limited Purpose FSA - $3,050; Dependent Care FSA - $2,500 (married and filing separate) and $5,000 (married or single parent). For Commuter Benefits, the IRS allows a monthly maximum of $300 for transit and $300 for parking in 2023. How do I change my FSA payroll deduction amount? FSA plans only allow changes to your election (that would in turn update your payroll amount) with a qualifying life event, such as the addition or loss of a qualifying dependent, or during your annual open enrollment period. Contact the Human Resources department at your employer for more information. Will my FSA funds rollover at the end of the year? Health FSAs are use-it-or-lose-it plans. Some FSAs offer a rollover feature that allows you to roll over up to $610 of unused FSA funds into the following plan year. Check to see if your employer offers this option. It’s important to plan accurately when selecting your contribution amount(s) and to ensure you know if your FSA plan allows a rollover. Who qualifies as a dependent? A person generally qualifies as your dependent for FSA purposes if you claim them as an exemption on your Federal tax return, even if they are covered under another health plan. Before the plan yearBefore the plan year begins, you’ll estimate your upcoming expenses, select a contribution amount (up to the annual maximum) and enroll in your plan. Pre-tax contributions will be deducted in equal installments from each paycheck.During the plan yearDuring the plan year, you’ll be able to quickly and easily make payments for eligible expenses using pre-tax dollars. The entire contribution amount will be available for you to spend right away. Yet you’ll fund the account with equal, tax-free deductions throughout the year—one in each paycheck. You’ll use just one prepaid benefit card to pay, whether you have one Voya Financial flexible spending account or multiple accounts. Receipts are required, and you’ll be able to submit them using the online portal or mobile app. If you forget or are unable to use the benefit card, don’t worry; you can still get reimbursed. Simply use the app or online portal to request a direct deposit to your account.At the end of the plan yearAt the end of the plan year, you’ll review your actual expenses and re-enroll.Use the mobile app or online portal to monitor your account and ensure you’re on track to spend all of your funds.Then, consider whether you spent more or less than your total contribution amount. You may use one or the other, or a combination of both, as long as you do not obtain pre-tax treatment and file for a tax credit on the same money. Benefit Strategies may also ask you to send in this documentation to substantiate that a reimbursement was for an eligible expense incurred during the plan year.Option 2: Federal Income Tax CreditYou are allowed to take a credit on your personal taxes, for the work-related child and dependent care expenses you incur. The maximum expense that may be considered is $3,000 for one qualifying dependent or $6,000 for two or more. However, only a percentage of these expenses may be deducted. The percentage depends on your adjusted gross income (or joint adjusted gross income if you and your spouse file a joint return.) You may take a credit of between 20% and 35% of your dependent care expenses, as shown on the chart below.

They are special cards, that provides you with easy access to your Flexible Spending Account(s) to pay your IRS qualified health and daycare expenses right at the point-of-sale on the date the services were incurred. The cards will only be accepted at specific healthcare providers such as physician and dental oces, pharmacies, hospitals, chiropractors, optometrists, etc., or dependent day care provider locations. The FlexExpress© Cards are issued by Benefit Strategies under the VISA system.How the FLEXEXPRESS© card works:Once your Flexible Spending Account is established, you will receive a set of FlexExpress© Cards at your home address, via US Mail. The FlexExpress© Cards can be used like a credit card to transfer funds from your Flexible Spending Account(s) directly to the health or day care provider, to pay your eligible out-of pocket expenses. How to Login to the Mobile App1. Use your username and password to access the consumer portal is entered to log into the app.2. First-time users will be prompted to enter their username and password and create a four-digit passcode when they open the app.3. If you have never logged into the system, you can create a username and password on the Mobile App. The New User? Set up your account button appears on the Mobile App login page.Helpful Tip: There is a ten limit maximum allowed attempts and usernames must be unique. When you attempt to change the username on the Mobile App, the system checks the uniqueness of the username. If it is not unique it is considered an unsuccessful attempt to change the username and is counted toward the ten limit maximum. If you exceed the allowed attempts to change the username on the Mobile App, access is locked, and you will be advised to contact us.

Filing Claims in the Mobile AppYou can file claims and submit associated receipt images.• If you have multiple plans to file against, you can tap the File A Claim button and then choose from a list of existing plans. If you have only one plan that can be filed against, you must tap the File A Claim button and then select a payee or fill out the claim form.Note: If you do not have a dependent set up, the app will display a message indicating that a dependent is needed to file a claim against the Dependent Care plan.Note: The Claim Submitted page also shows any denial letters were generated during claim submission.

Adding Receipts to Debit Card ClaimsThe Claims Requiring Receipts page displays the number of debit card claims requiring receipts, if applicable. This list does not display if there are no receipts required. This function is the same as the Action Required alert on the Consumer Portal home page.• By clicking on the line of a payment, you can expand the data to display additional details about the transaction.• Tapping the envelope icon displays the Claims Requiring Receipts page.The Claims Requiring Receipts screen displays all claims requiring receipts. This function is the same as on the Consumer Portal home page, where it displays the claim Receipt Status (Required or Overdue) or one or more repayments in a Pending status. The following information is displayed:• Claim Amount• Date of Service• Merchant/Provider• Recipient• Plan Name/Plan Year Name• Claim Status• Receipt StatusNew Receipt and View Receipts• If the claim receipt status is Required or Overdue, a New Receipt list item displays. Tapping this list item displays the New Receipt screen.• If there are one or more receipts associated to the claim, they display in the list. Tapping a receipt list item allows you to view the receipt image file.Android: The receipt image file is temporarily downloaded to the device and, when fully downloaded, the app attempts to find another app to view it. Adding Receipts to Claims ContinuedThe Receipt screen activates the device’s camera (if available) and allows a photo to be taken of a receipt and submitted. The first screen activates the camera and allows the picture to be taken. When taken, it is previewed on the second screen, where you can submit it or opt to retake it.1. Selecting photos will bring up your gallery of previously taken pictures and allow you to select one.2. Tapping the Upload Receipt (Android) or Receipts (iOS) link activates the device’s camera (if the device has a camera) and allows a photo to be taken of a receipt and submitted. The first screen activates the camera and allows the picture to be taken. When taken, it is previewed on the second screen, where you can Use or Retake it.3. Click Use to submit the receipt image. Submitting the receipt associates the receipt to the claim and makes it viewable on the mobile app.4. You can preview and remove a receipt image before uploading it from the mobile app. By tapping Attached Receipt, you have the option to view or remove the receipt.

Initiate Repayments from The Mobile AppTo initiate a repayment, the following conditions must be met:• You must have an active bank account.• A repayment cannot be partially paid.• You cannot repay provider repayments.If you have a repayment, you will see a new message in the Tasks section on the Mobile App Home page. The repayment details an amount will be visible on the Mobile App, but you will be directed to the Consumer Portal to make the actual repayment.1. Tap on the Action Required message.2. Tap on a claim in the Pending Repayments section to view the claim and repayment details.3. Tap Make Repayment.4. Tap Next to initiate the repayment process using the active bank account.5. Accept the Repayment Disclaimer agreement by selecting the "I have read..." option and tapping Agree.
